DO NOT GIVE A CREDIT CARD NUMBER ON THIS FORM.

Once your form is submitted, your CODA membership will be verified. If you are a non-member, Corliss Rose or Sandy Walker from CODA administration will call you for your credit card payment information. Your order will then be processed and your registration booked. Confrmation of your registration will be sent to you via email. Your badges will follow via US Mail. Registrations made after September 15th will require badge pick-up at the CODA check in booth at Pechanga.
